Clinical Pharmacists identify, resolve, and prevent drug-related problems. To optimize workflow, clinical pharmacists use a pharmacovigilance monitoring system, recommend and monitor patient drug therapy, and provide drug information. Clinical pharmacists interact with other health care providers to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective drug therapy.
